If you believe that a deposit is missing, please use the form below to submit screenshots of 401k contributions in your account. Please also provide screenshots of paystubs showing your deducted 401k amounts. You may redact account numbers or other identifying information. The union will not share sensitive information, but we will alert management and pursue corrections to missing contributions.
